Juice w/ Attitude CTS and receive a $50 Mail-In Rebate, Juice w/ Attitude CS and receive a $40 Mail-In Rebate, Evolution CTS, Insight CTS, Attitude CTS or EvoHT and receive a $25 Mail-in Rebate or Evolution CS, Insight CS, or Attitude CS and receive a $15 Mail-In Rebate. Purchase must be made between 06/01/2012 and 07/31/2012.
Send: 1) This completed form
2) Original UPC from the Product
3) The orginal dated, store-identified, cash register receipt with the purchase of the Edge product purchased between 06/01/12 and 07/31/12
